I If you have other insurance please inquire if we participate. You must bring your insurance benefit card for every visit to assure accurate processing of your claim. Co-payments are required at the time of visit.

If you are being seen for a work injury or injury resulting from a car accident, please let the receptionist know when you make the appointment so that adequate time is reserved for your appointment.
