HIT Project Coordinator
Planning & Development

Requirements:          Bachelor’s degree required, Masters in Public Health or related field preferred.  Experience developing, administering and analyzing contracts, agreements, reports and trainings.  Strong written and verbal communication skills along with excellent organizational and time management skills required.  Proficiency with computers and Microsoft Office Suite required.

Responsibilities:     Help in the development, implementation and management of HIT related grant projects.  Including but not limited to, the New York State Department of Health’s Health Workforce Retraining Initiative (HWRI) which supports the development and implementation of an Electronic Health Records Training Program to address the collective need for basic computer training and electronic health record (EHR) orientation among medical providers and support staff in the New York City area.  Manage the budgetary and reporting requirements for new and existing health information technology project grants.  Prioritize and allocate available project resources; review and evaluate project deliverables; assist in developing new project function elements, including researching, compiling and analyzing supporting data.  Participate in the planning and development of new health information technology related grant proposals.  Assist in the negotiation, development, and monitoring of contracts and agreements.  Perform complex administrative duties requiring oversight, attention to detail and analysis; perform strategic planning and financial analysis, including assisting in preparation of project budgets to include staffing and operational needs.  Promote HWRI Program by reaching out to PCHIC members and other CHC's through e-mails, phone calls and monthly meetings.  Oversee the dissemination of assessment tool that determines computer competence of CHC staff, analysis of completed assessments, and appropriate training sessions assignment based on the tool.  Schedule on-site trainings and reserve transportation for all training equipment to be brought on site the day of training.  Conduct trainings as needed.  Coordinate replication and distribution of training materials.  Meet regularly with the training team to assess and refine program training sessions.  Identify and develop new trainings as needed.

Nutrition/Diabetes Education Coordinator     
Bronx Health REACH/CEED

Requirements:    Masters Degree in Nutrition, Public Health, or related degree.   May consider BA degree in health education if the candidate has considerable experience in nutrition and diabetes education.  Minimum of three (3) years experience providing nutrition services (clinical or educational).  Experience implementing nutrition programs for low-income, ethnically diverse populations preferred.  Strong background in diabetes and nutrition counseling and/or diabetes and nutrition program development and implementation preferred.  Bilingual (Spanish/English) preferred.  

Responsibilities:    Oversee the health promotion components of the program and provide ongoing support to the Coalition in the areas of diabetes nutrition, exercise, and general health promotions.  Provide ongoing training and education related to nutrition and diabetes to Coalition members.  Oversee nutrition programs within the Faith-based Initiative, the after school and the Community Health Advocates program.
