Case Manager
Homeless Sites
Requirements: Bachelor's
degree in Social Work
or related field. Minimum of (1) one
year of case management experience, preferably in a medical setting. Bilingual Spanish preferred. Knowledge of Medicaid and Managed Care. Demonstrated strong verbal and written
communication skills. Proficient with
computer software, data entry experience a plus. Understanding
of the healthcare environment
in New York
a
plus. Knowledge
of Social Services agencies and resources in the NYC area.
Responsibilities: Work
with interdisciplinary team and provide care coordination services to
assist homeless adults at program outreach sites. Specifically,
facilitate Medicaid
enrollment, utilization of 340B and
Pharmacy Assistance Programs (PAP) programs and assist with specialty
care
referrals and follow up. Work with providers,
patients and specialists to facilitate referrals process for uninsured
patients. Screen
clients at selected sites for Medicaid eligibility. Provide
intensive tracking
and follow up to ensure that all required supporting
documentation is
obtained and coordinate application submission with facilitated
enrollment
staff. Also work identified patients providing intensive case
management to
link to housing and other services.
